1. Ensure the client is Independent in Daily living (can cook, clean, and manage personal care) 

2.Receiving SSI/SSDI, or has verified employment (3+ months)

3. NO current Drug or Alcohol Use (Drug and Alcohol-free program) 

4. Open to living in a shared or private room setting, with common shared spaces. 

5. Submit the intake form:

Be sure to include any special circumstances we should be aware of to better serve each and every client. 

Once the Intake form is received, a team member will follow up within 24 to 48 hours to schedule a phone screening and then an in-person appointment when space becomes available.

Note: We do not charge application fees or accept deposits. A Program fee of $300 must be paid upfront prior to move-in.
